What happens after I submit?

Our team will call during clinic hours to answer your questions and confirm a time and location. This form requests an appointment; your visit is booked once we confirm it with you.

Do I need to file a lawsuit to get care?

No. You can receive care without filing a lawsuit. Our team can help you understand your insurance and payment options before your visit.

How does $0 out-of-pocket care work?

Eligible auto insurance benefits, such as Medical Payments (MedPay), may cover your treatment costs. We bill your insurer directly. Coverage, remaining benefits and your treatment needs determine what you owe; $0 is not guaranteed for every patient. Our team will verify your benefits and explain any costs before treatment.

What if I don’t have MedPay or my benefits run out?

Ask our team about other insurance and payment options. Depending on your case, an attorney-lien arrangement may be available. A lien defers payment until your case is resolved; it does not make treatment free.

Do I need my insurance details now?

No. Just your name and phone number gets the conversation started. We can help you choose a clinic, discuss coverage and check appointment availability when we call.
