About This Policy

This policy explains what information Aim High Health ("we" or "us") collects through aimhighhealthco.com, including our appointment pages at aimhighhealthco.com/funnel, how we use it, and who we share it with.

Your Health Information

We are a healthcare provider. Health information we receive about you, including what you send us through this website, is also protected by HIPAA. Our HIPAA Privacy Policy describes how we may use and disclose that information and your rights under HIPAA.

Please do not use our website forms for emergencies. If you have a medical emergency, call 911.

Information You Give Us

  • Forms: when you contact us, request an appointment, refer a friend, or sign up for our newsletter, we collect what you enter, such as your name, phone number, email address, preferred location, the service you are interested in, and your message. If you refer a friend, we also collect the contact details you give us for them.
  • Appointment questionnaires: our appointment pages ask a few questions about your situation, such as where it hurts, how long it has been going on, when an accident happened, what you have already tried, and whether you have had imaging, and then ask for your name, phone number, email address, and preferred location. If you accepted analytics in our cookie banner and enter your name and phone number but leave the page without submitting, we save what you entered, marked as incomplete, so a Patient Coordinator can call you. If you did not accept, nothing is sent until you submit.
  • If you accepted analytics, with each form we also record how you reached our website: the page you first landed on, the website that referred you, and any campaign tags in the link you followed.

Information Collected Automatically

When you first visit, a banner asks whether to turn on our website analytics. If you click Reject, none of what this section describes runs, and the site and its forms still work. If you click Accept, our website platform records how visitors use the site: the pages you view, the website or link that brought you here (including any campaign tags in that link), the links and buttons you click (including phone and email links), how far you scroll, how long a page is open, when you submit a form, and the basic technical information every browser sends, such as your IP address and browser type.

It stores a random identifier in your browser, using a first-party cookie and local storage, so it can recognize return visits. When you submit a form, the platform links the email address or phone number in it to that identifier, and keeps them in your browser, so our staff can see how you found us and follow up on your request.

Who We Share Your Information With

We share information from this website only with service providers that work for us:

  • Sapt, which hosts and operates this website, our forms, website analytics, and patient messaging, along with the hosting and communications providers it uses to deliver those services.
  • Cloudflare, which delivers this website and measures how quickly its pages load.

We do not sell your information. We do not use advertising pixels (such as the Meta Pixel), advertising conversion tags (such as Google Ads), session-recording tools, or Google Analytics on this website, and we do not share your form submissions or your activity on this website with advertising platforms.

Embedded Content and Links

Our pages use fonts from Google Fonts. Pages with videos show a preview image from YouTube, and the video loads from YouTube in its privacy-enhanced mode only when you press play. Our location maps load map images from OpenStreetMap. Some links take you to other services, such as our online intake forms (ChiroTouch), Zocdoc for booking, and Google Maps for directions. When that content loads or you follow those links, your browser connects to those companies, and their privacy policies apply.

How We Contact You

We use the phone number and email address you give us to respond to your request and follow up about scheduling. You can ask us to stop contacting you at any time.

SMS Messaging Terms & Conditions

At Aim High Health, you can opt in to receive text message notifications. About these notifications:

  • Notifications may include appointment reminders, updates, and other information related to the services provided by Aim High Health.
  • Messaging frequency varies.
  • Standard messaging rates and data charges may apply.
  • You can opt out of receiving these SMS messages at any time by replying “STOP” or by contacting Aim High Health directly.
  • You can text “Options” for help information.

How Long We Keep Information

We keep information for as long as we need it for the purposes described in this policy, or longer when the law requires it, such as medical-records retention rules.

Data Security

We take reasonable measures to protect your information. No method of transmission over the Internet or electronic storage is completely secure, so we cannot guarantee absolute security.

Your Choices

You can change your analytics choice at any time with Cookie Settings at the bottom of the page; withdrawing it clears what the platform stored in your browser. You can also block or delete cookies and local storage in your browser settings. To request a copy of the information we hold about you from this website, or to ask us to correct or delete it, contact us. We may need to keep some information, such as medical records, to meet legal obligations. You can opt out of promotional emails using the unsubscribe link in them.

Changes to This Policy

When we change this policy, we will post the new version here and update the date at the top.
